Phase 1 – First call
Upon death of the planholder or assignee, the authorized representative shall call Eternal Plans to inform and request for memorial services. A representative of Eternal Plans will initiate the provision of memorial services with the accredited mortuary.Phase 2 – Preparation, Cosmetics and Casketing
The servicing mortuary shall perform the embalming, restoration of disfigured body parts (if any), application of cosmetics, and placement in the appropriate casket. The kind of casket will depend of the fund for memorial services.Phase 3 – Viewing and Final Arrangements
Facilities for wake shall be provided at home or mortuary. The number of days will depend on the plan type chosen.Phase 4 – Trip to the interment site
The mortuary transports the deceased to the interment site. The plan benefit covers 25 km radius from the viewing place to the interment site.Depending on the plan type chosen, please note the following relative to memorial service benefit:
The Memorial Services Benefit may also be used to render cremation services.
The Memorial Service Benefit may also be used for an assignee or a deceased other than the
planholder.Upon death of the planholder, if there is no accredited mortuary in the area or if the memorial service was not facilitated by Eternal Plans, the Memorial Service Benefit shall be released to the beneficiary in the form of Unrendered Service. The amount of unrendered service benefit will depend on the plan type bought.
If the Memorial Service Benefit (fund) is not sufficient for the desired (upgraded) memorial services, the bereaved will pay any variance to the accredited mortuary.

We advise planholders not to go directly to the mortuary, accredited or not, to ensure that the applicable memorial services are rendered.
Upon receipt of the advice or request for service a memorial service representative of Eternal Plans will initiate the memorial services with an accredited mortuary.
